System Safety Working Group SSWG

At the working level, system safety tasks are normally performed by a system safety working group (SSWG). One of the reasons that a system safety working group or a team approach is generally used in system safety efforts is that multiple talents and disciplines are required in order to provide for the first-time safe, efficient operation of the entire system (Fig. 2-6). 

Normally, a separate system safety working group (SSWG) is designated for each project. In some situations, however, one SSWG may successfully participate in multiple projects. 

Outline the recommended composition of a system safety working group (SSWG), and discuss the knowledge and skills that each member is expected to contribute. 

An example FHA analysis worksheet is shown in Figure 2.31. This particular FHA analysis worksheet utilizes a columnar-type format. Other worksheet formats may exist because different organizations often tailor their FHA analysis worksheet to fit their particular needs. The specific worksheet to be used may be determined by the SSP, system safety working group (SSWG), or the FHA customer.

SYSTEM SAFETY WORKING GROUP (SSWG) CHARTER... 

An accident analysis report may be prepared by the system safety working group and/or an independent accident investigation board. The recommended method for serious accidents is for the SSWG to prepare an accident analysis report based on its own investigation and upon the findings of the traditional accident report prepared by the accident investigation board.
