This document is intended as a typical guideline and reference book that may be applied at petroleum, petrochemical and chemical facilities. It is suggested that this document is used as a practical reference to prepare the safety review requirements for these and related industries, and their process safety management systems. 

The Nil had become concerned with the performance of the two licensees because of a variety of problems. These included the diminished quality of recent Periodic Safety Review (PSR) submissions, the foiline of the licensees to deliver promised PSR modifications, their failure to make long-term commitments in areas such as research, and the decreased quality of some technical advice. The Nil had followed up on these problems individually, but the frequency and consistency of the observed problems started to suggest a systemic underlying weakness arising from the restructuring. 

The most frequently used type of written reporting system is a suggestion program that empowers employees to provide written safety and health suggestions for review and potential incorporation into an employee safety and health program. 

When the inherent safety process has been expanded to review regular or routine operation, the team will look at all aspects of inherent safety to provide suggested improvements for both the existing facility and for the next plant. 

The comments column may contain additional descriptive information or actions/ recommendations. The recommendations, sometimes with more detailed explanations, can be summarized in the report to produce a list of action items or suggestions for improving the safety of the process. These results should be reviewed with management to assure that the findings are transmitted to those ultimately responsible for any actions. 

The reviewers simply meet in an informal fashion to examine the process equipment and operating procedures and to offer suggestions on how the safety of the process might be improved. Significant improvements should be summarized in a memo for others to reference in the future. The improvements must be implemented before the process is operated.
