Step 6 Realign systems, both enabling and sustaining 

Safety performance and organizational sustainability are closely linked. 

Many enabling elements are static—i.e., they reside in written documents and procedures but are not used regularly as part of the day-to-day activity of the working interface. For example, staff frequently fail to wash their hands despite the written policy and convenient opportunities to do so, just as they fail to report incidents or avail themselves of safety suggestion opportunities. 

Because of this behavior failure, high-functioning organizations institute positive mechanisms of behavior observation and feedback to ensure that systems are actually used and critical behaviors are performed consistently and reliably. (We ll talk more about behavior observation and feedback in step 7.)
