Each chapter starts with a description of the topic covered in the chapter. This is followed by a short example highlighting a reported incident involving a batch reaction system. The case study is followed by a listing of key issues and process safety practices unique to the topic. The issues and concerns presented in this book, as well as potential design solutions and sources of additional information are presented in the tables. This format concisely conveys the necessary and relevant information in a familiar and convenient format. The organization of the tables is described below. 

Polymerization was generally carried out in a freezer at —20" C under a 350 nm UV source. Among all the metals listed in Table 7.6, the MFE for the production of secondary tin is as much as 91% of the MFE for the production of primary tin. This unusual situation arises from the fact that tinned steel contains only 0.2-0.3% tin, and the processing of this source involves dissolution of the tin in a caustic solution, followed by an energy-intensive electrowinning operation to obtain metallic tin. An advantage is that the detinned steel scrap is suitable for steel manufacture. 

Most QI programs build upon an established quality assurance (QA) process. Quality assurance can be defined as a formal and systematic process in which problems in delivering health care are identified, solutions to the problems are developed and implemented, and follow-up monitoring then is carried out [30]. QA begins with problem identification. After possible sources of the problem are determined, solutions are developed and implemented. Then, the results of the intervention or solution are evaluated to determine whether or not the problem has been resolved. By incorporating a goal of continual improvement, a QA process can contribute to quality improvement activities. 

Therefore, criteria in the selection of an electrode reaction for mass-transfer studies are (1) sufficient difference between the standard electrode potential of the reaction that serves as a source or sink for mass transport and that of the succeeding reaction (e.g., hydrogen evolution following copper deposition in acidified solution), and (2) a sufficiently low surface overpotential and rate of increase of surface overpotential with current density, so that, as the current is increased, the potential will not reach the level required by the succeeding electrode process (e.g., H2 evolution) before the development of the limiting-current plateau is complete.
