Serious violation

For most processes, the optimum operating point is determined by a constraint. The constraint might be a product specification (a product stream can contain no more than 2 percent ethane) violation of this constraint causes off-specification product. The constraint might be an equipment hmit (vessel pressure rating is 300 psig) violation of this constraint causes the equipment protection mechanism (pressure relief device) to activate. As the penalties are serious, violation of such constraints must be very infrequent. 

Non-linear models, such as described by the Michaelis-Menten equation, can sometimes be linearized by a suitable transformation of the variables. In that case they are called intrinsically linear (Section 11.2.1) and are amenable to ordinary linear regression. This way, the use of non-linear regression can be obviated. As we have pointed out, the price for this convenience may have to be paid in the form of a serious violation of the requirement for homoscedasticity, in which case one must resort to non-parametric methods of regression (Section 12.1.5). 

The accident investigation evidence showed that this safe procedure was not followed specifically, the product takeoff valve was removed, the DEMCO valve was in the open position, and the lockout device was removed. This scenario was a serious violation of well-established and well-understood procedures and created the conditions that permitted the release and subsequent explosion. 

Regression analysis assumes that all error components are independent, have a mean of zero and have the same variance throughout the range of POM values. Through an examination of residuals, serious violations in these assumptions can usually be detected. In the case of protocol violations, a team member must determine whether this resulted from the clinician s misunderstanding or misinterpretation of the protocol, indifference and sloppiness, inability to communicate necessary information to the patient, or, as happens in rare cases, fraudulent practices. It is very likely that when there have been serious violations or a substantial number of dropouts, the study will have to enroll more patients and be extended beyond the originally projected time frame. 

The most serious violation of Rule 2a is the nonstereospecific formation of 1,2-dimethylcyclopropane from irradiation (24) of cis-2,3-dimethylcyclobutanone ([50c]) and trans-2,4-dimethylcyclobutanone ([50t]). Until very recently no serious exceptions to Rule 2b, regiospecificity, were reported. The report of Miller (77) that cyclobutanones fused to bicycloheptene and bicyclo-octene skeletons do not lead to major products expected from cleavage at the most substituted a-carbon, represents a serious violation of Rule 2b. 

Many companies, in their internal training, include information about the anti-kickback statute. The Office of the Inspector General in the Department of Health and Human Services (HHS) enforces the antikickback law. HHS administers the Medicaid and Medicare programs. Any serious violations are referred for potential criminal prosecution to the Department of Justice administrative sanctions also are possible. 

Agree or not, marijuana is currently federally designated Schedule I. So if children choose to consider it no more than a harmless, recreational drug, they need to know that because of its current classification, involvement with marijuana is often treated as an extremely serious violation of the law. Prosecution of the federal violation sometimes takes precedence over state statutes that apply. In the past several decades, millions of people have been arrested for possession, use, and sale of marijuanamore than half a million in 1966 alone. Apparently, when a substance that people want is outlawed, some will risk becoming outlaws to get it, whereas others will not. 

Heavy disintegration. This level of decay can be identified when whole areas of the structure have been assimilated, and only dissociated portions are left. The physical matrix has been seriously violated so that it no longer works as a unit. Degradation is apparent through the loss of mechanical properties, as well as through evidence from byproducts (frass, macerated cells, surface hyphae) and often audible sounds as dissociated sections rattle or rub together. 

Wastewater treatment plants have also beea reported to m achieving the designed treatment efficiencies. Based on die random sampling242 plants m ten states, the U.S. General Accountmg Office (GAO), found that 87% of the plant samples were m violation of their permits and 31% were in serious violation of then permits (69). 

Laboratory equipment usage, maintenance, calibration logs, repair records and maintenance procedures should be examined. Data would be suspect if it were generated from equipment known to be defective. Continuing to use and release products on the basis of such equipment would be a serious violation of GMP. The difficulty in identifying all of man s rights, and in elucidating them clearly and methodically, is not the only problem involved in a work of this nature. It must be done in a way that is not too lengthy or too detailed, so that each right would be explained in terms that make any serious violation of that right clear, open to simple demonstration and understandable to all.
