The authors of the Guidelines for Risk Based Process Safety (KBPS) (CCPS, 2007a) described the need for constant vigilance as the price of maintaining an effective process safety management system. An operator not only must be vigilant (aware of both past and current performance), but must not assume that current performance will be maintained, much less improved, without intentional evaluation of critical parts of systems and their performance. Performance measurement and metrics are a critical part of the RBPS system. 

One must recognize that the main function of a measure of safety performance is to describe the safety level within an organization. .. in effect, then, measures of safety performance mnst... describe where and when to expect trouble and must provide guidelines concerning what we should do about the problem [p. 49]. 

Measurement is a prerequisite for control and prediction. The main function of a measure of safety performance is to reveal the level of safety effectiveness. A second purpose is to provide continuous information concerning the safety state. Measures of safety performance must help prevent, not just record, incidents. They must indicate where hazards-ielated incidents will likely occur and provide guidelines concerning the appropriate preventive initiatives. 

Leadership by the top management is most important. They shall lead by personal involvement to set example and shall inspire aU members of the organisation (seniors and juniors) to meet the goals of the company towards TQM approach and should instil values for safety, pollution control, and quality. The leadership shall provide guidelines and directions to subordinates that are understood by aU and create well-defined systems, as well as methods for their implementation and performance measures for achieving those goals.

Define the temperature failure criteria for the material. The experimental formulas were blended and extruded. The extruded products were measured for heat deflection temperature using ASTM D 648 as a guideline. Multiple measurements at various heating rates were conducted. An appropriate engineering safety factor was applied to the data. A critical temperature failure criteria was defined as 70°C for these particular experimental formulas. 70°C was considered the maximum sustained temperature the extrusions could withstand and still provide acceptable engineering performance.
