Safety Culture Change Management Functions

Poor management control or lack of management control is what invariably leads to accidents, which in turn cause loss. The key to a positive safety culture, a weak safety culture, or a change in safety culture is management s commitment, involvement, and leadership of the safety drive.

The safety culture criteria depicted in Table 9.1 can be understood as the goals or ideal end states of patient safety management and they can be used as a frame to define monitor indicators. Organisational activities can be viewed from the point of view of how they contribnte to these six ideal states. The control functions in Table 9.2 on the other hand can be used for selecting drive indicators that facilitate change in the right direction. 

The material in this long chapter may not cover the needs of every chemical plant and every petro-chemical plant, but the ten or fifteen Management of Change procedures developed by major corporations and reviewed by me did not seem to exactly fit the needs or culture of my own organization. Trevor Kletz has said many times that improper plant modifications have been a major cause of chemical plant accidents. I have been working in a process safety function for three decades and my experiences have been similar. It just seems appropriate to repeat the first three paragraphs of the second section of this chapter, as a fitting close. 

In summary, the SUNflower, or pyramid, approach to understanding the effects of policy decisions consists of different layers so that causal relationships among changes in the layers can be modelled. This can be seen as a core element of the SUNflower approach. Accordingly, road safety management can thus be seen as part of the bottom layer of the pyramid structure and culture .
