Your employees can provide a tremendous amount of information. Take the time to talk to them—and be sure to listen Asking for their honest input will engage them in the process and lead to a higher level of safety awareness. You will likely hear several practical ways they believe the job hazards and job processes affecting them can be eliminated or at least reduced.

Organizational motivational practices will influence employee safety behavior. Research has demonstrated that organizations that exercise humanistic management approaches have better safety performance (Cohen 1977 Smith et al. 1978 Qeveland et al. 1979). These approaches are sensitive to employee needs and thus encourage employee involvement. In addition to the motivating factors, safety and health professionals should be aware that the safety and health function, unlike other operational functions, can be influenced by other factors beyond the safety and health professional s control, which can impact the safety and health function and motivation of the workforce. For example, if the operation is anticipating a reduction in the workforce (RIF), this anticipation could result in higher injury rates, increased workers compensation costs, and related factors. The safety and health function also possesses the seasonal or operational ebb and flow of the specific operations. For example, the number of slip-and-fall injuries increases during the months of November through February when snow is on the ground. Safety and health professionals should be keyed in to the unique features of the specific operations and workforce.

Retrospective reviews of medical records aim to assess the nature, incidence and economic impact of adverse events and to provide some information on their causes. Adverse events are defined as an unintended injury caused by medical management rather than the disease process that results in some definite injury or, at the very least, spent on additional days in hospital (Box 4.1). Definitions are critical in patient safety and one has to be constantly aware of differences in terminology. For instance, a study by Andrews et al. (1997) in the United States showed a 17.7% rate of serious adverse events in a surgical unit, much higher than most other studies. However, their definition of adverse event was different from that usually employed and they used observation rather than record review, as most other studies do.
