Risk management common cause events

ABSTRACT According to relevant documents and payment in the form of questionnaires, 224 cases of the domestic excavation accident in past two decades are collected in this paper. Based on processing and analysis of accident cases, the common risk events and factors are determined in the construction of deep excavation. Meanwhile the relativity between risk of quality and risk of safety and risk of investment is analyzed according to causes and losses of the accident, and the graph of macroscopic logical relation established between the risk of three objective dimension, which provides a reference for comprehensive risk management excavation construction process and basehne data for the theoretical analysis of risk. 

It is not the intent of this Guideline to deal in depth with facility security issues. However, effective fire prevention in a processing facility depends on people in addition to systems to detect developing fires and other incidents and to detect unauthorized intrusion into the facility. Intruder-caused vandalism, damage, spills, releases, or fires are not common, but are a credible threat. The potential fire prevention and protection requirements to manage the risk of security events from terrorism need to be considered in the overall fire protection system design. 

Each adverse effect, e.g. fire in suppliers facilities or the bankruptcy of the main business partner, causes certain consequences and problems for other links in the supply chain. The effect of the consequences of risk along the supply chain is defined as disruption. Such a turn of events is commonly compared to the domino effect. One event triggers another, and the consequence of one event becomes the cause of another. Disruptions in supply chains are "unplanned events that may occur in the supply chain which might affect the normal or expected flow of materials and components" (Svensson 2000, pp. 731-749). An adverse effect that leads to a long gap in the flow of processes in the supply chain is a critical disruption of the supply chain.
