One of the more methodologically rigorous studies on the utility of TCA/antipsychotic combinations in treating PMD was completed by Spiker et al. [1985). In this study, 54 patients who met criteria for depression with psychotic features on the Schedule for Affective Disorders and Schizophrenia [Endicott and Spitzer 1978) and by Research Diagnostic Criteria [Spitzer et al. 1985) were randomly selected to treatment with amitriptyline alone, perphenazine alone, or the combination of two drugs. (1981) studied 49 patients with mild to moderately severe depressions in a psychiatric outpatient department and 39 patients from a general practice in Edinburgh, i.e. a total of 88 patients, of whom 64 (14 men, 50 women) completed the study. In the United States, the Research Diagnostic Criteria (RDC) (19) and the DSM-IV (8) both provide clear inclusion and exclusion criteria for a current episode ( Table 9-2). Evaluation of past episodes can be made using the Schedule for Affective Disorders and Schizophrenia—Lifetime Version (SADS-L) ( 20) or the Structured clinical Interview for DSM (21). In other countries, the Present State Exam (PSE) (22) can reliably distinguish mania from other disorders. Table 9-3 reviews the various clinical presentations of primary bipolar disorder and their related DSM-IV diagnoses ( 23) (see also Appendix A, Appendix G, and Appendix H). 

Solubility is a critical functional characteristic because many functional properties depend on the capacity of proteins to go into solution initially, e.g. gelation, emulsification, foam formation. Data on solubility of a protein under a variety of environmental conditions (pH, ionic strength, temperature) are useful diagnostically in providing information on prior treatment of a protein (i.e. if denaturation has occurred) and as indices of the potential applications of the protein, e.g. a protein with poor solubility is of little use in foams). Determination of solubility is the first test in evaluation of the potential functional properties of proteins and retention of solubility is a useful criterion when selecting methods for isolating and refining protein preparations (1). Several researchers have reported on the solubility of extracted microbial proteins (69,82,83,84). In many instances yeast proteins demonstrate very inferior solubility properties below pH 7.5 because of denaturation.
