The RALES trial randomised 1663 patients with stable heart failure to either placebo or spironolactone (New England Journal of Medicine 1999 341 709). All patients were maintained on their optimised therapy that included ACE inhibitors. After 2 years of follow-up the trial was terminated prematurely due to a 30% reduction of mortality in the spironolactone treated patients both progressive pump failure and sudden death were significanly reduced. Gynaecomastia or breast discomfort occurred in 10% of treated patients (1% in controls), but significant h) erkalemia occurred in surprisingly few patients. RALES was not adequately powered to decide whether the action of spironolactone is additive to that of a p-blocker. 

Row-rate trials have been carried out from 111 to 684 m3/d water (700 to 4,300 BWPD] for a single 60-mm [2 4-in ] unit with about a 1200-kPa [174-psi] driving force. Efficiency starts to fall off at flow rates less than 159 m3/d water [1,000 BWPD] on the low end of the flow rate curve, again because there is insufficient flow lo support the vortex motion. It is also worth noting that the minimum flow rale threshold for the 60-mm [2.4-in.] cyclone is... 

The Lipinski Rule-of-Five almost certainly represents the first real attempt to identify a drug-like filter for molecular databases. Lipinski analysed the compounds within the World Drag Index that had progressed to Phase II clinical trials or better and discovered a simple set of rales that have been used extensively since their publication in 1997. The LRoF states that a compound may have lower membrane permeability or poorer absorption if two or more of the rules are violated. These rules are a compound should have no more than 5 hydrogen-bond donors, no more than...

Although spironolactone has been available for more than 30 years, its efficacy and safety in patients with heart failure have only recently been recognized in the Randomized Aldosterone Evaluation Study (RALES), in which it reduced mortahty (1). Based on this and numerous smaller trials, the use of spironolactone, in conjunction with ACE inhibitors, other diuretics, and possibly beta-blockers or digoxin, represents a promising strategy for patients with severe heart failure. Its main adverse effects are hyperkalemia and antiadrenergic complications (SED-14, 675). 

The contribution of [his adult Life stage to the evolution of resistance had not been fully evaluated and adults are exposed to insecticide during aerial application and through contact with treated crops. From field trials of commercial application rales Daly and Fisk concluded that selective mortality in the adult life stage contributes to the evolution of pyrethroid resistance.

This prompted a review of their use by the UK Committee on Safety of Medicines (CSM) in 2003, which raled that for major depressive disorder (MDD) in children and adolescents under the age of 18 ... 

Apart from the valuable benefit of aldosterone antagonism in hypertension, MR blockade has been shown to substantially reduce both morbidity and mortality among patients with severe chronic heart failure (CHF) and post-myocardial infarction (MI) in clinical trials [17, 18]. The Randomized Aldactone Evaluation Study (RALES) has shown that 26 mg spironolactone on average per day on top of existing standard therapy [i.e. an angiotensin-converting enzyme (ACE) inhibitor, aspirin and a loop diuretic] given to patients with severe heart failure (New York Heart Association class III or IV, left ventricular ejection fraction <35%) results in 30%... 

The sample of patients selected for a clinical trial may not be representative of the entire population of patients with that disease. Patients entered into a trial usually are selected according to the severity of their disease and other characteristics inclusion criteria) or are excluded because of coexisting disease, concurrent therapy, or specific features of the disease itself (exclusion criteria). It always is important to ascertain that the clinical characteristics of an individual patient correspond with those of patients in the trial. For example, the Randomized Aldactone Evaluation Study (RALES) showed that treatment with the mineralocorticoid-receptor antagonist spironolactone was associated with a 30% reduction in death in patients with severe congestive heart failure. Hyperkalemia, a potential adverse effect, was seen only rarely in this study, which excluded patients with serum creatinine levels >2.5 mg/dL. With the expanded use of spironolactone after RALES was published, numerous patients, many of whom did not meet the RALES inclusion criteria, developed severe hyperkalemia. Therefore, knowledge of the criteria for selecting the patients in a trial must inform the application of study results to a given patient. 

The Rule-of-Five (Ro5) is one of the best known and widely accepted drag-likeness filters. The Ro5 was derived from an analysis of 2,245 drag candidates from World Drag Index (WDl) with assigned United States Adopted Name (USAN) or International Proprietary Name (INN) that are believed to have reached Phase n trials. It states that a compound violating any two of the following rales is likely to be poorly absorbed ... 

In our modeling, we used a square N x N lattice (N = 400-1600) with periodical boundary conditions. The states of square cells were set according to the rales determined by the detailed mechanism of the reaction (e.g., in the case of Pd(l 1 0) each lattice cell can exist in one of five states , COads, Oads, [ Osub], [COads Osub])- The time was measured in terms of the so-called Monte Carlo steps (MC step) consisting of x trials to choose and realize the main elementary processes. For an MC step, each cell was called once in the average. The probability of each step for the processes of adsorption, desorption, and reaction was determined by the ratio of the rate constant of a given step to the sum of the rate constants of all steps. 

One feature of the two-phase region can be determined by cubic equations. Maxwell s equal-area rale (which will be verified in Chapter 6) provides a graphical means to determine for a given T. It states that the saturation pressure is the pressure at which a horizontal line equally divides the area between the real isobar and the solution given by the cubic equation. Such a construct is illustrated in Figure 4.12, where the equal areas above and below the isobar fix the value for P. This procedure can be achieved by trial and error. If a higher saturation pressure were predicted, the upper area would be too small. Conversely, too low a value for P would make the upper area too large.
