Steroid Hormones and Neurosteroids. Steroids (qv) can affect neuroendocrine function, stress responses, and behavioral sexual dimorphism (78,79) (see Steroids). Mineralocorticoid, glucocorticoid, androgen, estrogen, and progesterone receptors are localized in the brain and spinal cord. In addition to genomic actions, the neurosteroid can act more acutely to modulate the actions of other receptors or ion channels (80). Diarrhea is common soon after starting octreotide but usually resolves within 2 weeks without specific treatment. In a randomized, double-blind, placebo-controlled trial in 203 mostly postmenopausal women with locally recurrent or metastatic breast carcinoma, all of whom were also taking tamoxifen, and who had estrogen-receptor positive and/or progesterone-receptor positive tumors, octreotide was added to the basic treatment in 99 cases (31). The adverse events experienced by 10% or more of the patients and attributed to octreotide were gastrointestinal diarrhea (53%), nausea (16%), and abdominal pain (11%) diarrhea occurred in only 11% of the controls.

Endometriosis-associated pain is secondary to structural and/or inflammatory causes. The lesions may cause pain by compression of nerve fibers. Increased pressure within endometriomas (cysts within the ovary) has been linked to dyspareunia. Endometrial lesions also generate local inflammation with prostaglandin release and increase the risk of developing adhesions. Endometrial lesions contain estrogen and progesterone receptors, and symptoms may correlate with the cyclic release of hormones during the menstrual cycle.
