A study of the records of 83 patients with primary brain tumours who were treated with procarbazine between 1981 and 1996 showed that 20 of them had procarbazine hypersensitivity reactions. Of these 20, 95% had also taken antiepileptics compared with 71% of those not developing hypersensitivity. In addition, there was a significant dose-response association between the development of hypersensitivity reactions and the serum levels of the antiepileptics used (phenytoin, phenobarbital, or car-bamazepine, with or without valproate). It was suggested that the enzyme-inducing antiepileptics may increase the metabolism of procarbazine to metabolites causing hypersensitivity. This may also explain why the incidence of procarbazine-induced hypersensitivity is higher in patients with brain tumours, who commonly receive seizure-prophylaxis therapy. ... [Pg.656]
