Stenting preoperative

The first stent implantations described by Dohmoto et al. in 1991 and Spinelli et al. in 1992 were endoscopic placements of self-expanding metallic stents for palliative treatment of colorectal cancer. Only later the preoperative stent placement for single-step surgery using radiologic techniques under fluoroscopy was first described as a technical note in two patients by Tejero et al. in 1994. The fundamental idea behind using preoperative stenting... [Pg.61]

In selected cases stents may also be used for preoperative decompression of obstruction in acute diverticulitis (Baron et al. 1998 Binkert et al. 1998 Knopfle et al. 1998 Camunez et al. 2000 Tamim et al. 2000). The goal is to relieve the patient immediately from symptomatic ileus and to treat the inflammation with antibiotics prior to surgery. Since it is often difficult to distinguish diverticulitis from a malignant tumour as the cause of obstruction even on endoscopy, preoperative stenting should be performed anyway to allow cleansing prior to resection. [Pg.61]

In the two review articles by Mauro et al. (2000) and Zollikofer et al (2001) compiling series with more than ten patients between 1996 and 2000 with a total of 117 and 140 patients, respectively, undergoing preoperative stent placement for acute ileus, a clinical success was achieved in 89%-96%. The technical success rate for crossing the lesion with a guidewire and successive stent placement was 90% and 89%, respectively. The majority were treated with Wallstents (mainly of the oesophageal, vascular or Enteral ... [Pg.67]

According to Binkert et al. (1998) cost savings of up to 29% can be achieved in preoperative stent placement for colonic carcinoma. However, no completed randomised prospective study has compared preoperative stenting with standard surgery in patients with potentially resectable primary colorectal cancer and obstruction. Currently such a study is under way in the United States (Baron 2001). [Pg.73]

Treatment of large bowel obstruction using selfexpanding metal stents is now well-established and widely disseminated. Stenting is used in patients with incurable disease for definitive palliation, or preoperatively for patients where curative resection is possible (Camunez et al. 2000). [Pg.171]
