Many accidents occur because the operator or maintenance person does not understand the hazards involved or has not been instructed on the precautions to be taken. The policy document should identify the main hazards within the company with advice on which rules must be obeyed while carrying out a hazardous task. General rules should also be included to cover items such as untidy work areas, replacement of guards, the use of protective cloth-ing/equipment where appropriate, safe working practices in handling goods and materials, etc. It is essential that on the introduction of new products, processes, operations or plant and equipment that any hazards associated with these are brought to the attention of all concerned. 

The policy document should be reviewed at regular intervals to ensure that any changes from the introduction of new legislation (i.e. regulations, codes of practice, information concerning safe working procedures) are implemented.

The policy documents advocating the setting up of specialist multidisciplinary teams, whether they are concerned with mentally-handicapped people, alcohol, or other drug misusers, all made recommendations regarding catchment area and staffing. In practice, the teams which have been set up have been found to vary so much in size, disciplinary mix, and population served, that it is difficult to know what is actually meant by the terms CMHT, CAT, or CDT. 

The VMP should be a summary document and should therefore be brief, concise, and clear. It should not repeat information documented elsewhere but should refer to existing documents such as policy documents, standard operating procedures (SOPs), and validation protocols/reports. The documentation format is illustrated in the VMP.
