Oxytocin (Pitocin, Syntocinon) causes milk release (letdown) by stimulating contraction of the myoepithelial cells of the milk ducts in lactating mammary glands this forces milk from the alveoli of the breast. Oxytocin release is stimulated by suckling and by auditory and visual stimuli, such as a baby s cry. Oxytocin is available as a nasal spray, which is used as an aid to lactation when milk ejection is impaired. [Pg.683]

Oxytocin (Pitocin, Syntocinon) is a cyclic 8-amino acid peptide that is synthesized in the paraventricular nucleus of the hypothalamus and transported within hypothalamic neurons (in association with neurophysin) to the posterior pituitary for storage. Its mechanism of action involves the direct stimulation of oxytocin receptors found on the myometrial cells. Oxytocin circulates unbound in the plasma, where it has a half-Ufe of approximately 15 minutes. It is primarily inactivated in the kidneys and liver. [Pg.718]

Once the cervix has been ripened, or in patients who present with a favorable, soft cervix, labor induction can proceed with IV oxytocin (Pitocin ). This is a synthesized form of the same peptide composed of nine amino acids that is synthesized by the hypothalamus and secreted by the posterior pituitary. Although it will cause contractions beyond the late second trimester of pregnancy, these contractions do not always lead to labor. Labor also can be initiated or augmented by rupture of the amniotic and chorionic membranes, which is thought to up-regulate oxytocin receptors on the uterus and release prostaglandins.
