Patients discharge

Joan has been serving as the clinical coordinator for the department of pharmacy in a 500-bed hospital for more than 9 years. She provides clinical services to patients admitted to the general medicine service, manages clinical programs for the department, and is responsible for the continued professional development of 12 clinical pharmacists. Joan is a member of the pharmacy and therapeutics committee, the nutrition support committee, and an ad hoc committee to improve the patient discharge process. She also chairs the hospital s smoking-cessation campaign.

E Discontinue abciximab, aspirin, and heparin and give a platelet transfusion. Because abciximab is associated with thrombocytopenia, platelet counts should be monitored carefully. The next question was whether the pharmacy discharge profiles of our patients, sorted by ethnicity, would show differences in doses needed to produce clinical improvement sufficient to permit patient discharge. 

Most patients are used to providing this type of information for their doctor s office visits, but may question the pharmacist s need to inquire. The pharmacist should explain that updated information will assist in providing better care for the patient. For example, when a patient s insurance does not cover the medication the patient was prescribed upon hospital discharge, the cost may prevent the patient from taking the medication. Obtaining insurance information prior to patient discharge as part of the history can prevent this type of problem. 

Some research projects promoted or initiated by the Foundation are currently ongoing. A European research consortium, with the participation of different countries, is proposed to study the influence of pharmaceutical care in the control and treatment of minor diseases from community pharmacists, and it has submitted to the European Community in Brussels asking for economic support. Such research will be coordinated by the School of Pharmacy of the University of Manchester, United Kingdom. A project promoted by the European Society of Clinical Pharmacy is currently studying the number of drug related problems (DRP) identified by community pharmacists on patients discharged from the hospital. A research study on the number of DRP identified by pharmacists in patients admitted to emergency services of hospitals was initiated...

The Pharmacy Continuity of Care Project, a study by the Faculty of Pharmacy at the University of Sydney, promoted the use of patient discharge forms that were sent by the hospital pharmacist to (1) the community pharmacist and (2) case conferences between these two individuals and the patient s general practitioner. ... 

Changing parenteral antimicrobials to oral regimens in carefully selected patients is now relatively common practice and allows for less expensive hospitalizations and earlier patient discharges.

The Patient Demographic module contains the graphical controls to add/edit/search demographic data of a patient. Using this module one can add/modify visits, document upload/download documents, add/ modify transfers or other type of information (allergies, antecedents, and medical history), health insurance, and patient discharge.

Individuals exposed to vapor and exhibiting miosis only or miosis and rhinorrhea only do not usually require antidote treatment and will resolve without medical intervention (Cannard, 2006 ATSDR, 2007), but they should be observed. If rhinorrhea is problematic in these vapor-only cases, then ATSDR (2007) advises intramuscular atropine (0.05mg/kg pediatric 2.0 mg adult) to relieve signs, followed by patient discharge. If eye pain/headache or nausea is problematic in vapor-only...
