A fixed-combination ointment containing oxytetracy-cline and polymyxin B is available for topical ocular use (see Table 11-6). The Centers for Disease Control and Prevention recommends ophthalmic ointments containing a tetracycline or erythromycin as an effective alternative to silver nitrate for prophylaxis of gonococcal ophthalmia neonatorum. Class III variants (formed at 0.1% of the frequency of class I) show increased resistance to oxytetracyciine, and elevated levels of both a brown pigment and oxytetracy-cline. The majority of these variants also show a reproducible large-scale DNA rearrangement, which probably included deletion and a low-level reiteration (three cw lour copies) of a DNA fragment.
