The N-terminal methionine residue of protein can also be employed for selective PEGylation using aldehyde-terminated PEG via a reductive amination reaction, because the N-terminal primary amine has a lower pAa of 7.8 than other amines such as lysines, whose pZa is 10.1 [7]. After reaction with aldehyde-terminated PEG at low pH, the resultant imine is reduced with sodium cyanoborohydrate to provide PEGylated protein (Fig. 4) [8, 9]. This technique was used for the production of Neulasta, which was approved for use by the FDA in 2002 [10]. [Pg.119]

Dosage form Neulasta is supplied in 0.6 ml prefilled syringes. Each syringe contains pegfilgrastim 6 mg (based on protein weight) and excipients in a sterile solution. [Pg.158]

Route of administration Neulasta is administered by subcutaneous injection. A physician may determine that a patient or caregiver can safely administer Neulasta at home. [Pg.158]

Recommended dosage and monitoring requirements The recommended dosage of Neulasta is a single subcutaneous injection administered once per chemotherapy cycle. Neulasta should not be administered in the period between 14 days before and 24 hours after administration of cytotoxic chemotherapy. [Pg.158]

Drug interactions No formal drug interaction studies between Neulasta and other drugs have been performed. [Pg.158]

The less-frequent dosing of Neulasta means that patients will require fewer painful injections and fewer office visits. A clinical trial investigator observed, This approval... [Pg.160]
