For onychomycosis, relief of symptoms is slow. The infected nail will need months to grow out. The practitioner should advise the patient not to become frustrated by the slow resolution. Despite the slow progress, the antifungal agent is curing the infection. The practitioner should also advise the patient that even after the infection is cured, the nail may not look normal. ... 

Assess the patient s symptoms to determine if selftreatment with OTC antifungal therapy is appropriate or whether the patient should be evaluated by a practitioner. Exclusions for self-treatment include infection of nails or hair, unsuccessful initial treatment, worsening condition, signs of secondary bacterial or systemic infection, or chronic medical conditions such as diabetes, immunosuppression, or impaired circulation. 

A key factor affecting the length of time before the onset of symptoms as well as the severity of the symptoms is the part of the body that is exposed to the agent. Apart from mucous membranes, the regions of the body that are the most sensitive to vesicants are warm, moist areas, and areas with thin skin such as the face, armpits, inside of the elbow, genitalia, neck, skin between the fingers, and the nail beds. The least susceptible body regions are the palms of the hands, soles of the feet, front of the knee, and outside of the elbow. 

Psoriatic arthritis is a distinct clinical entity in which both psoriatic lesions and inflammatory arthritis-like symptoms occur. Distal interphalangeal joints and adjacent nails are most commonly involved, but knees, elbows, wrists, and ankles may also be affected.

Jaundice (also called icterus) refers to the yellow color of skin, nail beds, and sclerae (whites of the eyes) caused by deposition of bilirubin, secondary to increased bilirubin levels in the blood (hyperbilirubinemia, Figure 21.10). Although not a disease, jaundice is usually a symptom of an underlying disorder. 

During initiation of the zidovudine therapy, the common side effects include severe headache, nausea, emesis, fatigue, malaise and myalgia, but these symptoms diminish with time. Other side effects include nail pigmentation, esophageal ulceration, hepatitis, neurotoxicity and bone marrow suppression. 

By May of 1975 the number of Yusho patients had risen to 1291 (ref. 136, pp. 40ff). Common symptoms of toxicity were the characteristic acnei form eruption and eye discharges, along with swelling of the upper eyelids previously described, hyperemia of the conjunctiva, hyperpigmentation of skin, mucous membranes and nails (ref. 146). 

Thallium, atomic no. 81 This element is better known for its poisonous properties and links with almost undetected deaths and Agatha Christie -type murder mysteries.2 Thallium compounds are used in some countries as cheap insecticides, particularly for killing cockroaches. Intake of this element causes stomach pains, vomiting and nausea, painful soles and palms of hands, limb weakness, double vision, involuntary eye movements, hallucina tions, characteristic hair loss and white lines across the nails. The symptoms are often mis-diagnosed as other diseases. Treatment is with the chemical Prussian Blue... 

Timolol may also elicit dermatologic signs and symptoms that include rashes, alopecia, urticaria, and discoloration of nails. Other systemic effects reported after topical timolol treatment include myasthenia gravis and retroperitoneal fibrosis. When treating a nursing mother, clinicians should also be aware that topically applied timolol may be excreted in breast milk.
