Although creatinine concentration should be stable from day to day, it is a function of the amount of creatinine entering the blood from muscle, its volume of distribution, and its rate of excretion. Because the first two are usually constant, changes in the serum creatinine level would usually be a result of a change in the glomerular filtration rate (GFR). Abrupt cessation of glomerular filtration causes the semm creatinine to rise by 1-3 mg/dL daily. The blood urea nitrogen also rises with renal dysfunction, but is influenced by extrarenal factors as well. [Pg.1680]

Mechanical restriction caused by chest bellows malfunction may result from chest wall or skeletal deformity, loss of neuromuscular function, fibrosis of the pleural space, and abdominal overdistension causing upward displacement of the diaphragm, as well as decreased diaphragm movement. The most common pulmonary function pattern seen in these patients is a decrease in TLC and VC with only a slight decrease in RV. The RV is maintained in these diseases because lung compliance remains normal. The Dlco is normal or only minimally reduced, and the Dlco a (corrected for alveolar volume) is normal. The RV/TLC ratio is often increased in patients with restrictive chest bellows disease. Patients with neuromuscular disease also have reduced respiratory muscle function with a reduction in their MIP. [Pg.500]

Isoflurane is a respiratory depressant (71). At concentrations which are associated with surgical levels of anesthesia, there is Htde or no depression of myocardial function. In experimental animals, isoflurane is the safest of the oral clinical agents (72). Cardiac output is maintained despite a decrease in stroke volume. This is usually because of an increase in heart rate. The decrease in blood pressure can be used to produce "deHberate hypotension" necessary for some intracranial procedures (73). This agent produces less sensitization of the human heart to epinephrine relative to the other inhaled anesthetics. Isoflurane potentiates the action of neuromuscular blockers and when used alone can produce sufficient muscle relaxation (74). Of all the inhaled agents currently in use, isoflurane is metabolized to the least extent (75). Unlike halothane, isoflurane does not appear to produce Hver injury and unlike methoxyflurane, isoflurane is not associated with renal toxicity. [Pg.409]

The corticosteroids are administered with caution in older adults because they are more likely to have preexisting conditions such as congestive heart failure, hypertension, osteo-poros s and arthritis which may be worsened by the use of such agents The nurse monitors older adults for exacerbation of existing conditionsduring corticosteroid therapy. They are stimulated when the tidal volume exceeds 1 1. Nerve impulses are transmitted by the vagus nerve to the medullary respiratory center and inhibit the inspiratory neurons.
