Multiple sclerosis acute exacerbations

Multiple sclerosis (acute exacerbations) - 200 mg daily for a week, followed by 80 mg every other day or dexamethasone 4 to 8 mg every other day for 1 month. 

Corticotropin is used for diagnostic testing of adrenocortical function. This drug may also be used for the management of acute exacerbations of multiple sclerosis, nonsuppurative thyroiditis, and hypercalcemia associated with cancer. It is also used as an anti-inflammatory and immunosuppressant drug when conventional glucocorticoid therapy lias not been effective (see Display 50-1).

We also know that with multiple sclerosis, as time goes on, the exacerbations become more acute and the remissions less so. It becomes progressively more severe as time passes that s why the 14-year period is so significant in this particular case. The significance of the remarkable results presented is multiplied by the total time involved or, looking at it another way, multiplied by a total lack of time involved in this particular case. Particular attention should be given to the frequency of therapy and the total time of therapy.

Many patients with NMD develop CO2 narcosis with supplemental oxygen. Hypoventilation is often first recognized during a respiratory infection when bronchial mucus plugging triggers acute respiratory failure. Ventilatory failure can also develop suddenly or over a period of hours or days in patients with acute cervical myelopathies, Guillain-Barre syndrome, myasthenia gravis, acute poliomyelitis, or exacerbations of multiple sclerosis.
