For oral administration, the water-soluble preparation menadiol sodium phosphate, is used in patients with hepatic disease, especially biliary obstruction. The usual dose is 10 mg daily. Alternatively, phytomenadione tablets may be used in those patients who do not have impaired fat absorption. 

PhyUoquinone is vitamin Ki, menaquinones are vitamin K2, and the synthetic compounds menadione and menadiol are vitamin K3. In addition to menadione itself, menadiol diacetate (acetomenaphthone) is used in pharmaceutical preparations, and two water-soluble derivatives, menadione sodium bisulfite and menadiol sodium phosphate, have been used for administration of the vitamin by injection and in patients with malabsorption syndromes that would impair the absorption of menadione, phylloquinone, and menaquinones, which are lipid soluble. 

Menadiol sodium phosphate (vitamin Kj, Synkavit), the S3mthetic analogue of vitamin K, being water-soluble, is preferred in malabsorption or in states in which bile flow is deficient. The main disadvantage is that it takes 24 h to act, but its effect lasts for several days. The dose is 5-40 mg daily, orally. 

Menadiol sodium phosphate in moderate doses causes haemolytic anaemia and for this reason it should not be given to neonates, especially those that are deficient in glucose-6-phosphate dehydrogenase their immature livers are unable to cope with the heavy bilirubin load and there is danger of kernicterus. 

H5 oprothrombinaemia due to intestinal malabsorption syndromes. Menadiol sodium phosphate should be used as it is water-soluble. 

Menadiol Sodium Diphosphate, USP. Menadiol sodium diphosphate, letrastxlium 2-methyl-1.4-nuphthalene-diol bis(dihydrogen phosphate), tetrasodium 2-mcthylnuph-thohydroquinone diphosphate, vitamin K4. is a white hygroscopic powder, very soluble in water, giving solutions with a pH of 7 to 9. It is available in ampuls for use subcutaneously, intramuscularly, or intravenously and in tablets for oral administration. Unlike the other vitamin K analogues, menadiol oral absorption does not depend on the presence of bile. Once absorbed, it is converted to inenadione. 

Acquit coagulation defects are more common, and may be due to liver disease (since bile is required for absorption of vitamin K), dietary deficiency of vitamin K, or ingesUon of oral anticoagulant agents. Most of these can be treated by giving vitamin K and its congeners, menadiol sodium phosphate or phytomenadione.
