Melatonin headache

Ramelteon is a melatonin receptor agonist selective for the MTj and MT2 receptors. The dose is 8 mg at bedtime. It is well tolerated, but side effects include headache, dizziness, and somnolence. It is not a controlled substance. 

Melatonin appears to be well tolerated and is often used in preference to over-the-counter "sleep-aid" drugs. Although melatonin is associated with few adverse effects, some next-day drowsiness has been reported as well as fatigue, dizziness, headache, and irritability. Melatonin may affect blood pressure as both increases and decreases in blood pressure have been observed. Careful monitoring is recommended, particularly in patients initiating melatonin therapy while taking antihypertensive medications. 

Four of six children with pre-existing severe neurological disorders had increased seizure activity within 2 weeks of starting oral melatonin 5 mg at bedtime (12). Seizure frequency returned to baseline after treatment was stopped, and increased again after rechallenge with melatonin 1 mg. A convulsion during melatonin treatment, which recurred when medication was continued, has been reported to the WHO database but not published (5). Headache, which recovered after melatonin was withdrawn, has also been reported in a few cases (5).

Melatonin can cause some adverse effects. People should contact their health care provider if they have difficulty sleeping or feel too sleepy, develop a headache, chills, or itching, feel confused, or have a fast pulse after taking melatonin. Individuals who will be undergoing surgery should tell the anesthesiologist that they have been taking melatonin. 

Melatonin can cause unpleasant side effects, even when taken for a short period of time. The use of melatonin has been linked to sleepiness, confused thinking, rapid heartbeat, chills, headache, inability to sleep, and itching. People who experience these symptoms when using melatonin should contact their health care provider.

Zolpidem, chemically unrelated to benzodiazepines or barbiturates, acts selectively at the y-aminobutyric acid (GABA )-receptor and has minimal anxiolytic and no muscle relaxant or anticonvulsant effects. It is comparable in effectiveness to benzodiazepine hypnotics, and it has htde effect on sleep stages. Its duration is approximately 6 to 8 hours, and it is metabolized to inactive metabolites. Common side effects are drowsiness, amnesia, dizziness, headache, and GI complaints. Rebound effects when discontinued and tolerance with prolonged use are minimal, but theoretical concerns about abuse exist. It appears to have minimal effects on next-day psychomotor performance. The usual dose is 10 mg (5 mg in the elderly or those with liver impairment), which can be increased up to 20 mg nightly. Cases of psychotic reactions and sleep-eating have been reported.

Toxicity and drug interactions Sedation and next-day drowsiness and headache have been reported. Melatonin may suppress the midcycle surge of LH and should not be used in pregnancy or in women attempting to conceive. Since it may decrease prolactin levels, melatonin should not be used by nursing mothers.

Circadin is a prolonged-release tablet containing melatonin 2 mg. It has received European Medicine Agency approval for treatment of primary insomnia. Adverse reactions are uncommon, but include headache, pharyngitis, back pain, and weakness. There do not appear to be withdrawal effects.
