Liver Doppler ultrasound

Ultrasound, particularly Doppler ultrasound, provides an indication of blood flow through the liver and whether that flow is reversed. This is another way of identifying whether first-pass metabolism is likely to be affected by collateral blood flow bypassing the liver. The scan may also show whether the liver is large and inflamed (hepatitis) or small and knobbly (cirrhosis).

Typical metastatic liver tumors have a thick hypoechoic rim with a relatively hyperechoic center, known as the huU s-eye sign (Fig. 16.10a) however, this finding cannot he applied to all metastatic liver tumors. Penetration of the normal vascular structure through the tumor proven hy color/power Doppler, ring-shaped enhancement in the arterial phase, and loss of enhancement in the postvascular phase as determined hy contrast ultrasound, are helpful findings for the diagnosis of metastatic liver tumors.

Renal fracture is usually more difficult to characterize by grey scale ultrasound or Doppler (PiETRERA et al. 2001) injection of intravenous contrast medium can help approach that diagnosis (Fig. 25.5). Of course, the ultrasound examination of the abdominal cavity should be complete. For example, in cases of a right kidney lesion, it is common to find an associated liver lesion.

Power Doppler is more sensitive in revealing venous flows within hemangiomas. Recent papers have highlighted the potential of contrast enhanced harmonic ultrasound (US) to characterize liver lesions. In hemangiomas, the absence of intratumoral vessels in the arterial phase and peripheral nodular enhancement in the portal phase are the most typical patterns and were observed in 76% and 88%, respectively (Fig. 8.2) (Isozaki et al. 2003). The sensitivity, specificity, and accuracy of diagnosis based on this combination of enhancement pattern were 88%, 99%, and 98% (Isozaki et al. 2003).
