Finally, various anesthetic patches are available, such as Lidoderm, although their efficacy in achieving anesthesia before procedures has not been studied. Lidoderm patches contain 5% lidocaine and are approved for treatment of postherpetic neuralgia. Up to three patches can be used at one time for a maximum of 12 hours per day.

Evidence-based treatment guidelines recommend Lidoderm patch and Pregabalin as first lines of treatment for neuropathic pain associated with PHN. 

A recent randomized double-blind controlled study examined Lidoderm patch efficacy for relief of post-surgical pain after unilateral knee arthroplasty... 

There is evidence of the usefulness of low doses of lidocaine, such as obtained by using the Lidoderm patch, in the termination and early relief of migraine headache that is preceded by an aura. This application is based on lidocaine s effectiveness in the suppression of clinical and electrographic manifestations of seizures at low blood concentrations (below 5 pg/mL).

Each Lidoderm patch may be used only once for up to 12 hours/day. Lidoderm patches should be applied to the most painful area, only if the skin is intact. No more than three patches are to be apphed at a time. Average blood concentration achieved with this dosing is between 0.13 and 0.25 pg/mL of lidocaine, depending on the site of application (Table 69.2). Lidocaine toxicity coincides with a lidocaine blood level at or above 5 pg/mL. 

Patients with known allergic reactions to methyl-paraben or propylparaben may develop allergic reactions to the lidoderm patch. Severe hypersensitivity-types of reactions to lidoderm are rare. 

Application of a lidoderm patch for longer than 12 hours/day... 

Caution should be exercised with the disposal of used lidoderm patches, since about 95% of its lido-caine content could still be found in each used patch. It is therefore imperative to discard used patches after folding them so that the medication side of the patch is covered in order to avoid accidental ingestion by small children or pets. 

Lidoderm is a 10 cm x 14 cm patch containing 50 mg of lidocaine per gram of adhesive material. Each patch contains a total of 700 mg of hdocaine, the only active ingredient.
