Toxic effects Toxic effects, potentially serious, may be related in frequency and severity to dose or frequency of administration, but have been seen at all doses. These effects can occur at any time during therapy follow patients closely. Most adverse reactions are reversible if detected early. When reactions occur, reduce dosage or discontinue drug and take appropriate corrective measures this could include use of leucovorin calcium. Use caution if therapy is reinstituted. [Pg.1973]

It is used as leucovorin calcium (calcium folinate). It is 5-formyl derivative of tetrahydrofolic acid and it acts as an antidote to folic acid antagonists like methotrexate or pyrimethamine which inhibit the enzyme dihydrofolate reductase. [Pg.397]

Reduced folates are co-factors for the 5-fluorodeoxy-uridine monophosphate-thymidilate synthetase reaction. Leucovorin (calcium fohnate) therefore potentiates the toxicity of 5-fluorouracil, and fatal adverse effects have been reported in patients over 65 years of age receiving high-dose treatment with leucovorin simultaneously with fluorouracil. This has led some groups to recommend that initial dose levels of fluorouracil should be lowered by 20% and that therapy be stopped temporarily at the first sign of distal gastrointestinal adverse effects (SEDA-15, 414). [Pg.1435]
