Melanocortin peptides are potent anti-inflammatory agents displaying beneficial effects in diseases ranging from cardiovascular to arthritis to obesity to name a few. Within an inflammatory context, they have the ability to switch off early production of cytokines and at later stages they increase levels of anti-inflammatory proteins that lead to the resolution of the host inflammatory response potentially restoring homeostasis to the tissue. Early research showed that Pb exposure could increase sensitivity to bacterially-derived endotoxins [63] as well as increase production of the pro-inflammatory cytokine tumor necrosis factor-alpha (TNF-a) by macrophages [64-67], Studies in several species indicate that Pb boosts production of TNF-a both immediately following adult exposure and in later life following gestational exposure. Flohe and coworkers [67] reported that Pb-induced elevation in TNF-a production is sensitive to both protein kinase C signaling as well as protein production. Not only can the production of TNF-a be elevated following exposure to Pb, but also the expression of the receptor for TNF-a (TNF-R) is elevated [68], Therefore, the combined effect of elevated cytokine production by macrophages as well as increased receptor expression would be expected to contribute to problematic inflammatory responses. 

These molecules interact with the leukocyte integrins and thus are important during the later stages of the inflammatory response, which are independent of selectin function. The major molecules present on endothelial cells are shown in Table 3.3. 

The same principle was later applied by scientists at Immunex to enhance the pharmaceutical properties of the tumor necrosis factor receptor (TNFr), which, like CD4, is rapidly cleared from blood. The hybrid TNFr-Fc molecule (-150 kDa), which is now known as Enbrel, is used to reduce the inflammatory responses of rheumatoid arthritis [33,34]. The hybrid TNFr-Fc bas extended the plasma half-life of TNFr from several minutes to 190 hours. 

As for vesicant-injured skin, the inflammatory response appears to be biphasic. This is based upon our years of experience in addition to others who reported that inflammation played a minor role in the initial events of SM-induced cutaneous injmy, but had much greater importance at later stages (Papirmeister et al, 1991c). In a more recent examination, Cowan and Broomfleld (1993) argued that inflammation may be more signiflcantly involved in the vesication event than previously believed, with inflammatory cells and mediators contributing directly to the formation of the primary lesion.
