Selected scores have been proposed for stratifying risk after MI. These scores have been derived either from clinical trials (TIMI, PURSUIT, GUSTO, etc.) or from registries and cohort studies (PREDICT, CCP, etc.). The majority of them divide the ACS into two groups with and without ST-segment elevation (STE-MI or STE-ACS vs NSTE-MI or NSTE-ACS). This classification is very useful for a better approach of treatment. The GUSTO score includes QRS duration and ECG (Hathaway et al., 1998a,b) prior MI (Table 8.4), and the PREDICT score uses other ECG parameters (ECG severity score) that include ST, Q wave and branch block criteria (Jacobs etal., 1999 Table 8.5).

At the fifth level of the Jacob s ladder classification, the full information of the KS orbitals is employed, i.e. not only the occupied but also the virtual orbitals are included. The formalism here becomes similar to those used in the random phase approximation (Section 10.9), but very little work has appeared on such methods. Inclusion of the virtual orbitals is expected to significantly improve on, for example, dispersion (such as van der Waals) interactions, which is a significant problem for almost all current functionals. 

Table 6.1 shows an overview of commonly used functionals given by their acronym, and placed in the Jacob s ladder classification. One may furthermore differentiate the functionals based on their use (or lack) of experimental data for assigning values to the parameters in the functional forms. The non-empirical ones such as the PW86,... 

Although there are exchange-correlation functionals that do not fall into this classification, they are mostly functionals correcting the above functionals for some sort of physical factor. Such physical factors for functionals are introduced in Chap. 6. In this chapter, let us focus on functionals conforming to a standard paradigm in which LDA and GGA functionals are expanded by additional terms. These standard functionals are often arranged hierarchically, as in Jacob s ladder ( the ladder to Heaven in the Old Testament, Book of Genesis, Fig. 5.2) (Perdew et al. 2006).
