The comparison of a hybrid NORM drug versus a comparable combination therapy has rarely been studied in vivo for example, a cocktail of NSAID + ISDN (isosorbide dinitrate) compared to an NO-NSAID. Isosorbide is available in two forms, isosorbide dinitrate (ISDN) and isosorbide mononitrate (ISDN). ISDN is rapidly metabolised in the liver to the active mononitrate. ISDN is only available in oral form. An intravenous formulation of ISDN is available, but is only used for the treatment of acute heart failure. [Pg.147]

Compared with placebo, the combination of hydralazine and isosorbide dinitrate (ISDN) reduced mortality in patients receiving diuretics and digoxin (but not ACE inhibitors or /3-blockers). However, another trial comparing the combination with an ACE inhibitor found that mortality was lower in the ACE inhibitor group. Adverse effects (primarily headache and gastrointestinal complaints) with combined hydralazine-ISDN were common, limiting their use in many patients. Patient compliance also was an important issue because hydralazine-ISDN therapy was given four times daily in these trials. The oldest class of clinically used NO donors are the organic nitrates (R-ONO2) (Fig. 20.3), which are nitric acid esters of mono and polyhydric alcohols, including glyceryl trinitrate (GTN A.l), pentaerithrityl tetranitrate (PETN A.2), isosorbide dinitrate (ISDN A.3), and isosorbide-5-mononitrate (ISMN A.4).
