Inspection team witnesses

Sample collection will generally be conducted by the inspected State Party (during investigations of alleged use (IAU), the inspection team may collect the samples itself), however, the IT may collect the sample(s) if agreed in advance between the inspected State Party and the IT [VA.II.52], The IT has the right to conduct sample analysis using its own approved equipment or it may witness analysis performed by the inspected State Party [VA.II.53]. This leads to two different concepts for on-site analysis as laid out below.

The next item in the loop is a decision point for possibly introducing the use of inductive reasoning methods into the deliberations. If the deductive process continues to indicate progress, then additional facts are procured or the logic tree is restructured. For example, one witness stated a particular valve was open, yet the post-incident inspection found it to be closed. The team must be careful to ensure that the valve is closed because of the actions taken prior to the incident, and not as a result of post-event response activities. The position of this particular valve may be a critical item in determining which of two scenarios is the more probable case. The incident investigation team would then initiate a short-term action item to conduct a mini-investigation to resolve this question. 

Validation Team A well-defined validation team with a well-written description of responsibilities is required and assures the adequate realization of the validation tasks. A validation team should be composed by different responsibilities responsible-of-validation team, team leader, archive manager, test coordinator, quality assurance member, tester, and witness. The responsible-for-validation team elaborates and approves the VMP, protocols, and reports. The team leader should be responsible for the computer system validation and utilization. An archive manager is responsible for the management of all computer system validation documents. The test coordinator is responsible for the computer system test and coordinates the elaboration and operation of tests for evaluating the performance of the computer system. A quality assurance member is required to periodically inspect and train the personnel and review all the validation documents. The tester is responsible for the execution of the tests required to perform the validation protocol. The witness is responsible for observing and reviewing the operations of the tester. 

A multi-disciplined team interviewed witnesses and performed tests, and the evidence was followed to a sound conclusion. Investigators sent the explosion meters (flammable gas detectors) to the instrument shop for examination. The welding torch was also inspected. Experts found both the torch and the meter behaved satisfactorily. There was speculation that some aquatic life deteriorated and formed methane, but that was not the case. They concluded that the fuel was dissolved methane from the well water system that was routed to the pump seal. Trace amounts of dissolved methane (about 0.02 percent by volume) accumulated in the well water stream flowing through the 3/8-inch-diameter stream on the...
