Hypertension program

This analysis of the SHEP database confirms the findings of a systematic worksite hypertension program (358). In 7978 treated patients with mild to moderate hypertension, cardiovascular disease was significantly associated with serum uric acid (HR = 1.22 Cl = 1.11, 1.35), controlling for known cardiovascular risk factors. The cardioprotective effect of diuretics increased from 31% to 38% after adjustment for serum uric acid.

The treatment goals for withdrawal from ethanol, cocaine/ amphetamines, and opioids include (1) a determination if pharmacologic treatment of withdrawal symptoms is necessary, (2) management of medical manifestations of withdrawal such as hypertension, seizures, arthralgias, and nausea, and (3) referral to the appropriate program for substance abuse treatment. 

Insulin resistance has been associated with a number of other cardiovascular risks, including abdominal obesity, hypertension, dyslipidemia, hypercoagulation, and hyperinsulinemia. The clustering of these risk factors has been termed metabolic syndrome. It is estimated that 50% of the United States population older than 60 years of age have metabolic syndrome. The most widely used criteria to define metabolic syndrome were established by the National Cholesterol Education Program Adult Treatment Panel III Guidelines (summarized in Table 40-2).

Accelerated hypertension - Nicotine therapy constitutes a risk factor for development of malignant hypertension in patients with accelerated hypertension. Inhaler therapy should be used with caution in these patients and only when the benefits of including nicotine replacement in a smoking cessation program outweigh the risks.

The issues of whether hyperuricemia is an independent risk factor for cardiovascular disease and the clinical relevance of the rise in serum uric acid caused by diuretic treatment are controversial (SED-14, 660 351). With these difficulties in mind, drug treatment programs do surprisingly well at keeping people on the road to recovery. The NIDA says the treatment of addiction is as successful in 2002 as the treatment of other chronic diseases such as diabetes, hypertension, and asthma. 

Six drugs (including nimodipine) used together in a program of hypertension therapy were determined simultaneously by HPLC [24], The tablets were extracted with methanol, and felodipne was added as an internal standard. A column (25 cm x 4 mm i.d.) of Jasco Metaphase ODS silica (5 pm) was used with a mobile phase (flow rate of 1.5 mL/min) of 10 mM phosphate buffer of pH 4.5/acetonitrile (1 1). Detection was effected at 250 nm, and the linear operational range was found to be 25-3200 ng/mL.
