Greater occipital nerve blockade is a procedure used in headache treatment, despite the fact that controlled evidence for its efficacy is limited. One report of the occurrence of cutaneous atrophy, hyperpigmentation and alopecia in four patients who underwent this blockade with triamcinolone and lidocaine has been published [29 ]. Triamcinolone injection is associated with cutaneous atrophy, especially in superficial injection sites. The authors suggest the use of alternative steroid preparations like methylprednisolone and betamethasone because they might be more appropriate for this kind of blockade because both are more soluble than triamcinolone. The risk of cutaneous complications also depends on the depth of the injection, with deeper injections being associated with lower risk. [Pg.609]
