Glomerulonephritis drug-induced

A drug-induced systemic lupus erythematosus (SEE) with proliferative glomerulonephritis has also been described in patients treated with D-penicillamine [111, 157]. Systemic lupus erythematosus syndrome is induced in approximately 2% of patients treated with D-penicillamine [112,158]. Unlike other forms of drug-induced systemic lupus erythematosus, anti-double-strand DNA anhbodies and/or hypocomplementemia are seen in D-penicillamine-induced systemic lupus erythematosus syndrome [111, 156]. Nephropathy is rare in D-penicillamine-induced systemic lupus erythematosus syndrome [111]. Walshe [112] reported that 8 patients developed the serological change of systemic lupus erythematosus of 120 patients with Wilson s disease treated with D-penicillamine, but none of them showed nephropathy. [Pg.467]

Mechanisms of drug-induced kidney disease include immune-mediated toxicities (e.g., glomerulonephritis and allergic interstitial nephritis) and nonimmunologic-mediated toxicities which effect specialized characteristics of normal renal physiology. [Pg.871]

The kidneys are more sensitive than many other organs to drug toxicity. Immune-mediated, drug-induced nephrotoxicities include glomerulonephritis and allergic interstitial nephritis, either with or without nephrotic syndrome. The kidney is highly susceptible in part because of its large vascular surface area for exposure to circu-... [Pg.872]

Other adverse effects are caused by immunological reactions, of which the drug-induced lupus syndrome is the most common. Hydralazine also can result in an illness that resembles serum sickness, hemolytic anemia, vasculitis, and rapidly progressive glomerulonephritis the mechanism of these autoimmune reactions is unknown. The drug-induced lupus syndrome usually occurs after at least 6 months of continuous treatment with hydralazine, and its incidence is related to dose, sex,... [Pg.556]

Urinary tract Hydralazine has been associated with ANCA positive pauci-immune glomerulonephritis and pulmonary alveolar hemorrhage [105 ]. In this case anti-double-stranded DNA and antihistone antibodies were present in the absence of full classification criteria for drug-induced lupus-like syndrome. The symptoms and renal function improved after withdrawal of hydralazine and a short course of cyclophosphamide and prednisone. [Pg.332]

Immunologic A 50-year-old woman taking hydralazine for hypertension developed alveolar hemorrhage and anti-neutrophil cytoplasmic antibody (ANCA)-positive pauci-immune glomerulonephritis despite the absence of full criteria for drug-induced... [Pg.428]

Although rare, cases of renal lupus syndrome and pemphigus blisters have also been reported as a consequence of D-penicillamine-induced immune complexes (Ntoso et al., 1986 Bigazzi, 1988), as well as with other drugs. With renal lupus syndrome, secondary glomerulonephritis may result if granular IgG antibodies are produced and deposited on the basement membranes. In patients with pemphigus blisters, autoantibodies to the intercellular substance of the skin have been recovered from the sera, and dermal biopsies have demonstrated intracellular deposits or... [Pg.558]
