First aid logs

Every accident must be reported to an employer and minor accidents reported to a supervisor, safety officer or first aider and the details of the accident and treatment given suitably documented. A first aid log-book or accident book such as that shown in Fig. 1.34 containing first aid treatment record sheets could be used to effectively document accidents which occur in the workplace and the treatment given. Failure to do so may influence the payment of compensation at a later date if an injury leads to permanent disability. To comply with the Data Protection Regulations, from 31 December 2003 all first aid treatment log-books or accident report books must contain perforated sheets which can be removed after completion and filed away for personal security. 

First-aid log — Keep a record of all minor injuries, including any that require first aid. These injuries do not have to be recorded on the OSHA logs, and many companies feel that minor injuries are not worth the time or effort to investigate.

Looking at the OSHA 200 log and, where applicable, first aid logs, are there patterns of illness or injury that should have been analyzed for previously undetected hazards Is there an occupational nurse or doctor on the woiksite Are employees suffering from ordinary illnesses encouraged to see a nearby healthcare provider Are the records of those visits analyzed for clusters of illness that might be woik related ... 

Each Name of Company fadlity/worksite/jobsite will have adequate first aid supplies and certified, trained personnel available for the treatment of personnel injured on the job. It is also imperative that all treatments be documented in the first aid log. 

A first aid log should be maintained in the Name of Company first aid facility. This log should reflect the following information ... 

Neither OSHA nor ANSI requires that employers maintain a first aid log. 

It is always advisable to maintain a first-aid log, which provides a record of injuries that could have been potentially worse or recordable for OSHA record keeping. This is not required but is recommended as good business practice. A sample first-aid log is found in Figure 4.2. 

The format used to report first-aid cases or incidents can be similar to an accident report or can be a separate form. I recommend that your organization develop the form for these types of incidents in-house. Getting input from the people who will be filling out these forms for their development will likely increase the likelihood that they actually get filled out. The first-aid logs can resemble the OSHA 300 log. Any injury that resulted in a Band-aid, ice, a wrap, or medical treatment that is not considered to be recordable would be included on the first-aid log. The name or other identifier of the victim would be included, along with a brief description of the accident, treatment, status, and so on. 

The first-aid log is another important tool used in accident prevention. This type of log is not a requirement for OSHA. It is an in-house tool that, when studied carefully and acted on, will help reduce or eliminate accidents. If we believe in the pyramid effect, when we reduce the size of the bottom of the pyramid, which is reducing common minor incidents like first-aid incidents, this reduces the number of serious injuries.
