Ondansetron has also been investigated for the treatment of fatigue associated with primary biliary cirrhosis but results have been disappointing. A randomised, controlled crossover trial (n = 54) examined the effect of ondansetron 4 mg three times a day versus placebo for a four-week period, before being crossed over for a further four-week period. The study concluded that the use of ondansetron did not offer a clinically significant reduction in fatigue compared to placebo [20]. In a 9-week, single-blind, placebo-controlled pilot study in 72 patients with multiple sclerosis who took modafinil 200 mg/day for 2 weeks, there was significant improvement in fatigue compared with placebo run-in treatment (8). The most frequent adverse effects were headache, nausea, and anxiety, and these were rated as either mild or moderate.

A series of bathing solutions mimicking the intracellular environment in fatigued muscle have been studied by Godt and Nosek (1989). Variations in ATP, ADP, PCr, and Cr contents had only marginal effects on the tension development compared to decreases in pH and increases in Pj. 

An epidemiological study tested the effects of chronic exposure to jet fuel in factory workers (Knave et al. 1978). This study found significant increases in neurasthenia (i.e., fatigue, depressed mood, lack of initiative, dizziness, and sleep disturbances) in the exposed subjects when compared to unexposed controls from the same factory. In studies of tipranavir/ritonavir, the most frequent adverse reactions were diarrhea, fatigue, headache, nausea, and vomiting. Adverse reactions leading to discontinuation were reported by 7.8% of the tipranavir-treated patients and 4.9% of the comparator arm patients. 

The idea of reduced adrenal capacity as a possible model for PTSD has also been recently raised by Heim et ah, who concluded that low cortisol may not be a unique feature of PTSD, but may represent a more universal phenomenon related to bodily disorders, having an etiology related to chronic stress (Heim et al. 2000). There are numerous stress-related disorders such as chronic fatigue syndrome, fibromyalgia, rheumatoid arthritis, chronic pain syndromes, and other disorders that are characterized by hypocortisolism. In one study, Heim et al. showed decreased cortisol responses to low-dose DEX, but failed to observe blunted ACTH responses to CRF in women with chronic pelvic pain, some of whom had PTSD, compared to women with infertility (Heim et al. 1998). Since the data were not analyzed on the basis of the subgroup with and without trauma and/or PTSD, it is not possible to directly compare results of that study to other reports examining PTSD directly.
