Procedure request form, example

An example of a standard form for requesting a procedure change is shown in Figure 7-2. The titles shown on the form identify functions that can be performed by existing personnel. The procedure requestor is the person who requests the procedure development or revision. The procedure sponsor is the person who is responsible for ensuring that the procedure is accurate and up to date. The procedure coordinator is the person responsible for processing the procedure change requests. 

Simulation models like the one used in this example are very useful in practising Box-Wilson strategy. It can be obtained upon request from the author in the form of a Matlab, R or Excel VBA. For maximal learning, the user is advised to start the procedure at different locations. 

A questionnaire in the form of an Excel spreadsheet was distributed to partners. This requested details of the test procedure and examples of test data. The form of the questionnaire allowed easy coUatlon of returned data into an appropriate database. The database was then analysed to identify differences in test facilities, testing practice and the resultant data.
