Pregnancy (ergotamine s powerful uterine stimulant actions may cause fetal harm) hypersensitivity to ergot alkaloids peripheral vascular disease (eg, thromboangiitis obliterans, leutic arteritis, severe arteriosclerosis, thrombophlebitis, Raynaud s disease) hepatic or renal impairment severe pruritus coronary artery disease hypertension sepsis. The use of potent CYP3A4 inhibitors (ritonavir, nelfinavir, indinavir, erythromycin, clarithromycin, troleandomycin, ketoconazole, itraconazole) with dihydroergotamine is contraindicated. 

Drug-associated cerebrovascular events and fatalities Cerebral hemorrhage, subarachnoid hemorrhage, stroke, and other cerebrovascular events have been reported in patients treated with dihydroergotamine some have resulted in fatalities. Other vasospasm-related events Dihydroergotamine, like other ergot alkaloids, may cause vasospastic reactions other than coronary artery vasospasm. Myocardial and peripheral vascular ischemia have been reported with dihydroergotamine.

DIHYDROERGOTAMINE This substance, which is used in the treatment of migraine as well as hypotension, is classified in C04 and N02C, Ergot alkaloids. 

Dihydroergotamine, an ergot alkaloid with vasoconstricting properties (1 mg IM or IV), is used to prevent or abort vascular headaches, including migraine and cluster headaches (see also Figure 93).
