Elderly inflammatory bowel disease

This report highlights the risk of cardiovascular adverse effects with short courses of glucocorticoid therapy in elderly patients with inflammatory bowel disease, even with rather low-dosage regimens. Acute myocardial infarction occurred in an old man with coronary insufficiency and giant cell arteritis after treatment with prednisolone (SEDA-10, 343) but could well have been coincidental. [Pg.7]

Chronic diarrhoea may be due to enteric infection with parasitic or fungal organisms, drugs, malabsorption or inflammatory bowel disease. In severe cases it can lead to severe dehydration and electrolyte imbalance. This can be a particular problem in infants, young children and the elderly. [Pg.268]

The role of both T and B lymphocytes in a variety of disease states beyond transplantation has become increasingly important in the past decade. This is especially true of those diseases frequently referred to as autoimmune in their etiology, such as rheumatoid arthritis, nephrotic syndrome, systemic lupus erythematosus, inflammatory bowel disease, and so on. In addition, several other major diseases are also known to have a component of T- or B-cell-mediated pathogenesis, for example, atopic dermatitis, psoriasis, and asthma. Until very recently, the mainstay of therapy for these diseases was the corticosteroids, which were often less than satisfactory in efficacy and often associated with undesirable side effects, especially in growing children and the elderly. Thus, the search for new agents with different mechanisms of action and which did not have the same adverse event profile as conventional corticosteroids led to the subsequent evaluation of drugs such as tacrolimus and sirolimus to treat several of these diseases. [Pg.425]

The primary colonic lymphoma is usually found in middle-aged or elderly people. Males are twice as often affected as females. Common symptoms include abdominal pain, weight loss, and changing bowel habits with an average duration of about 4-6 months. Primary colonic lymphomas occur more frequently in the setting of inflammatory bowel disease and immunosuppression and are found most commonly in the cecum or the rectum (Breneton et al. 1983) (Fig. 13.17a-c). [Pg.170]

Codeine and morphine should be used with caution in hypotension, hypothyroidism, asthma (avoid during an attack) and decreased respiratory reserve, shock, prostatic hypertrophy, obstructive or inflammatory bowel disorders, diseases of the biliary tract, pregnancy and breastfeeding. They may precipitate coma in patients with hepatic impairment and as such, they should be avoided or a reduced dose used. In patients with renal impairment, the dose should be reduced or they should be avoided. If used in the elderly and debilitated, the dose should be reduced. [Pg.269]
