Long-term use - Tretinoin is a dermal irritant, and the results of continued irritation of the skin for greater than 48 weeks are not known. There is evidence of atypical changes in melanocytes and keratinocytes and of increased dermal elastosis in some patients treated with tretinoin 0.05% for longer than 52 weeks.

In conclusion, dermal effects, particularly chloracne, are the most commonly reported effects of 2,3,7,8-TCDD exposure in humans because they are easy to identify. Additional information is needed to determine the level and frequency of 2,3,7,8-TCDD exposure needed to cause chloracne and whether individual susceptibility plays a role in the etiology. Also, chloracne in humans indicates CDD exposure, but lack of chloracne does not indicate that exposure has not occurred. Other dermal conditions reported include hypertrichosis, hyperpigmentation, and solar elastosis.

Elastosis perforans serpiginosa starts as red umbUicated papules that coalesce to form annular (serpiginous or arcuate) lesions with clear centres. There are microscopically thickened coarse elastic fibers, extruding through narrow epidermal channels. 

In one patient the characteristic features of both of elastosis perforans serpiginosa and cutis laxa developed (276).
