Treatment plans are effective only if they help clients and families improve. To be able to determine progress, you need to review treatment plans periodically. Usually treatment teams will conduct weekly interdisciplinary staff reviews of a client s progress, and the staff s recommendations for improving care will be documented in the chart. Ideally, assessment measures would be incorporated into the treatment plan to assess baseline symptoms and response to treatment. Such measures include the Clinical Global Impression (CGI) scale, the ten-item Conners Global Index for parents (Conners-P) and Teachers (Conners-T) ( 60), and the SNAP (Swanson, Nolan, and Pelham) rating scale (61). The latter has also been used in surveys of school-aged children ( 62). The results have been used to produce age-adjusted cutoffs that distinguish normal from abnormal degrees of inattentiveness and hyperactive/impulsive behavior. Thus, this scale can also be used to aid in documentation of the diagnosis as well as to monitor response to treatment. 

Part II of the book outlines several mental-health diagnostic categories schizophrenia, mood disorders, depression, bipolar disorders, and specific anxiety disorders including generalized anxiety disorder and obsessive compulsive disorder. Each chapter provides a case example, consideration in diagnosis, and the interventions utilized. Medications used to treat these disorders and relevant psychosocial interventions are outlined. Each chapter emphasizes the need for accurate treatment planning and documentation and offers suggestions to facilitate this process.

The process was developed by Texas Alkyls (a joint company of Akzo and Hercules) and the U S. Library of Congress. A pilot plant was set up in 1988, capable of treating batches of 300 books, and plans to build two larger plants were announced in 1989.
