District audit

The Audit Commission for England and Wales promotes proper stewardship of public finances and helps local authorities and the NHS to deliver economic, efficient and effective public services. The Commission also owns District Audit, which it appoints to carry out many of the audits for which the Commission is responsible. District Audit operates as an arm s-length agency.

Whether or not an investigator seeks additional support or is accompanied by a partner, he or she is required to prepare a detailed report of the audit that will accompany the FDA-483 list of observations if one was issued. Fie or she will also recommend approval or suggest that FDA withhold approval based on the inspection. The district office and headquarters do not necessarily have to adhere to the recommendations however, they typically do.

When selecting facilities to audit, consideration should be given to common features (e.g., field supervisors, regulatory districts, facility design, systems and equipment, office management) to obtain a cross-section of practices for the facilities operated. 

I was appointed as NC for a critical care network in the southeast of England, which comprised of four district general hospitals with a total of 47 critical care beds. An audit identified the most difficult patient group to manage as those requiting LTMV. My role was to develop and implement pathways for care of the LTMV patient.
