Displaying the sequence of events

Various analytic tools have been developed for the purpose of displaying the sequence of events in a graphical format. They have different aims  

To help in checking that the relevant data about the sequence of events have been documented and that the data are logically coherent. 

To act as a communication tool, when groups discuss the sequence of events and causal factors. 

To provide coded data as input to accident databases.

Evaluation and organising of the information starts after the initial inspection of the site and interviews with participants and witnesses. The aim is here to establish a description of the sequence of events and to compare it with existing information to determine whether there are any inconsistencies. One of the methods for displaying the sequence of events presented in Section 13.4.3 may support this work. It is here important to identify and evaluate deviations from normal practices and failures of existing barriers and controls. The commission will often find it necessary to return to the scene or to conduct additional interviews to check and complement already-gathered information.
