This behavioural syndrome, rather emotively called learned helplessness", is widely believed to share many features of depression, not least because both culminate in psychomotor retardation and both are linked with experience of uncontrollable, unpredictable stress. Whether or not learned helplessness really is an analogue of depression remains controversial (Maier 1993). Nevertheless, escape deficits in rats are prevented by pretreatment with antidepressants from different generic groups. Other psychotropic agents, such as CNS stimulants and neuroleptics, are generally ineffective. [Pg.430]

During withdrawal, some patients may experience symptoms of steroid withdrawal (eg, joint or muscular pain, lassitude, depression) despite maintenance or even improvement of respiratory function. Encourage continuance with the inhaler, but observe for objective signs of adrenal insufficiency (eg, fatigue, lassitude, weakness, nausea and vomiting, hypotension). If adrenal insufficiency occurs, increase the systemic steroid dose temporarily and continue further withdrawal more slowly. The efficacy of benzodiazepines in most anxiety disorders has been proved through extensive clinical experience and controlled trials (Faravelli et al. 2003), although it is important to note that they are not effective at treatingpost-traumatic stress disorder or comorbid depression, and there is less evidence to support their use in obsessive-compulsive disorder (OCD). Their anxiolytic effects have an immediate onset and in contrast to many other drugs, they do not cause a worsening of anxiety when therapy is initiated. [Pg.473]
