Depo-medroxyprogesterone acetate

The benefit of depo-medroxyprogesterone acetate in dysmenorrhea is related to its ability to render most patients amen-orrheic within 1 year of use.17 This is an expected side effect. Since the pelvic pain of dysmenorrhea is related to the prostaglandins released during menses, in the setting of amenorrhea, the underlying cause of dysmenorrhea is removed. 

Observational data illustrate a reduction in dysmenorrhea from 60% to 29% with the levonorgestrel-releasing IUD after 3 years.17 As observed with depo-medroxyprogesterone acetate, this reduction is likely secondary to the increasing incidence of amenorrhea in users of this contraceptive device.

In 1992, the US FDA approved a long-acting contraceptive device containing synthetic progestin medroxyprogesterone acetate (MPA) hormones. Manufactured by Pfizer, Inc., it is sold under the trade name Depo-Provera . It is typically injected into a woman s buttocks or upper arm, and renders her temporarily sterile for up to 3 months. ... 

Medroxyprogesterone acetate and its metabolites are excreted in breast milk, so women who breastfeed should wait until 6 weeks post partum before starting Depo-Provera, when the infant s enzyme system should be more mature. Norethisterone enantate 200 mg (Noristerat) is shorter acting than Depo-Provera, 8 weeks, and is used to provide contraception after administration of the rubella vaccine, and until a partner s vasectomy has taken effect. It can also be used in the longer-term but only on a named patient basis.

Long-acting injectable analogues of the sex hormones (16.5) are also used. Depo Provera is injected every 3 months as a microcrystalline aqueous suspension. The active compound, medroxyprogesterone acetate, dissolves slowly as needed. Another compound that is injected once a month is an ester that probably hydrolyzes slowly.

Medroxyprogesterone acetate (Fig. 46.13), administered by injection (Depo-Provera), orally, or delivered via lUD, effectively suppresses the HPO axis, induces anovulation, and reduces serum estrogen levels (83). This prevents menstruation and endometrial implant growth. As a result, endometriosis-related pain is minimized in approximately 90% of the patients. Drug therapy selection should reflect the fact that pharmacological therapy is likely to be required on a chronic basis. The progestins are fairly well tolerated and relatively inexpensive, but they are not without adverse effects.
