Darvon , d-propoxyphene hydrochloride, is a synthetic, nonantipyritic, orally effective analgesic, with similar pharmacological activity and effects to codeine. Darvon is not a narcotic but can be substituted for codeine, and is useful in any condition associated with pain. Chemically, this analgesic is not analogous to codeine or to morphine.

Many other synthetic compounds such as dextropropoxyphene (D-propoxyphene) and methadone, for example, are potent narcotic analgesics (opioid agonists) and their use is controlled in most countries. Dextropropoxyphene is extensively metabolized by N-demethylation and by other routes. Methadone is metabolized largely by N-demethy-lation and hydroxylation. Dextropropoxyphene is thermally labile but may be measured together with nordextro-propoxyphene by HPLC with electrochemical oxidation detection methadone and some metabolites may also be measured using this same system. 

Tramadoi produces anaigesia by an opioid effect and by the enhancement of serotonergic and adrenergic pathways. The phenyipiperidine series of opioids (meperidine, tramadoi, methadone, fentanyi, D-propoxyphene) are very weak serotonin reuptake inhibitors. 

Nuclear magnetic resonance spectroscopy has been used to study self-ossociation in promethazine hydrochloride, in 2-butyl-3-benzofuranyl 4-[2-(diethylamino) ethoxy]-3,5-diiodo-phenyl ketone hydrochloride (SKF 33134A),40ond in d-propoxyphene hydrochloride. Florence has measured the properties of p-diethylaminoethyl diphenylpropylacetate hydrochloride (SKF 525-A) by light scattering, surface tension, and microelectrophoretic techniques. He suggests that caution should be exercised in the interpretation of enzyme inhibition results obtained with a compound of this type since it exhibits surface activity, and surfactants are known to exert an appreciable effect on certain enzyme systems.

The administration of CNS depressants as a result of failure to recognize salicylate intoxication may be disastrous (21 -). A case of combined overdosage with aspirin and d-propoxyphene with a fatal outcome has been reported (26 -).
