The cyanide antidote package (Taylor Phannaceuticals) consists of amyl and sodium nitrites (see p 476), which produce cyanide-scavenging methemoglobinemia, and sodium thiosulfate (p 505), which accelerates the conversion of cyanide to thiocyanate. 

A. Amyl nitrite. A component of the cyanide antidote package, 0.3 mL in crush-able ampules, 12 per kit. The drug may also be acquired separately in as-pirols. Note The ampules have a shelf life of only 1 year and may disappear because of the potential for abuse (as poppers ). 

B. Sodium nitrite parenteral. A component of the cyanide antidote package, 300 mg in 10 mL of sterile water (3%), two ampules per kit. 

C. The suggested minimum stocking level to treat a 70-kg adult for the first 24 hours is two cyanide antidote packages, or equivalent (one package should be kept in the emergency department). Available from Taylor Pharmaceuticals. 

A. Parenteral. As a component of the cyanide antidote package, thiosulfate sodium, 25% solution, 50 mL. Also available separately in vials and ampules containing 2.5 g/10 mL or 1 g/10 mL.

TLV equals that of hydrogen cyanide. 10% hydrocyanic acid solution has flash point of 2° C and a vapor pressure of 226mm Hg at 20 C. Log P octanot/water is estimated, cial first aid required in the event of poisoning antidotes must be available (with instructions). Unbreakable packaging preferred if breakable, keep in unbreakable container.
