Confined-space entry preparation

Employees stated that they had received confined space training and were prepared to perform confined space rescue, but they had not rehearsed rescue procedures. In addition, the confined space entry permit form for both contractors did not ask for all required information. For example, the form did not require documentation of the duration for the permit, the intended communications procedures for entry operations, or documentation of hot work performed during confined space entry operations. Completed permits did not contain documentation of hot work performed during confined space entry operations, even though hot work had been performed during such operations at least twice during the project. 

There may be many people involved with modifications while the rest of the plant is operating. Special extra emphasis on equipment preparation, lock out, barricading, confined space entry, etc., is vital. 

Training for confined space entry should be geared toward your involvement or likelihood of involvement with confined spaces and hazardous atmospheres. The training could range from how to use space heating devices and carbon monoxide buildup, to full blown entry/exit preparation and execution, air monitoring, respirator use, and rescue.

A permif sysfem for permif-required confined space entry is required by the confined space entry standard. An entry supervisor (qualified or competent person) must authorize entry, prepare and sign written permits, order corrective measures if necessary, and cancel permits when work is completed. Permits must be available to all permit space entrants at the time of entry and should extend only for the duration of the task. They must be retained for a year to facilitate review of the confined space program. 

Stringent preparation, isolation, air testing and other precautions are therefore essential and experience shows that the use of a confined space entry permit is essential to confirm that all the appropriate precautions have been taken.

Prior to beginning work in a confined space, workers should receive entry permits. A confined space permit system requires the identification of the space and an evaluation of the potential hazards. Supervisors and safety specialists must each agree and sign that all pre-entry evaluations and work and rescue preparations were completed and the space is ready for safe entry. 

Entering vessels. Work should not be performed inside a vessel or confined space until the vessel or confined space has been properly prepared and work procedures have been established, which will ensure the safety of workers. A confined space means a space that is large enough and so configured that an employee can bodily enter and perform assigned work, has limited or restricted means for entry or exit, and is not designed for continuous employee occupancy. 

This regulation requires that entry to a confined space to carry out work is prevented unless suitable and sufficient arrangements have been prepared for the rescue of persons in the event of an emergency.
