Clobazam Valproate

For simple and complex partial seizures and secondary generalized tonic-clonic seizures, the first line drugs are - carbamazepine, valproate and phenytoin. Second line drugs include - acetazolamide, clobazam, clonazepam, ethosuximide, felbamate, gabapentin, lamotrigine, levetiracetam, oxacarbamazepine, primidone, tiagabine, topiramate and vigabactin. 

For generalized absence seizures, first line treatment is with valproate or ethosuximide and second line treatment with acetazolamide, clobazam, clonazepam, lamotrigine, phenobarbitone and primidone. 

Myoclonic seizures are best treated with valproate but, as a second choice, with clobazam, clonazepam, ethosuximide, lamotrigine, phenobarbitone, piracetam or primidone. 

Epileptic-negative myoclonus status (almost continuous lapses in muscle tone associated with epileptiform discharges and interfering with postural control and motor coordination) rarely occur after rapid withdrawal of clobazam or valproate (10). 

Valproate, clonazepam, or clobazam may be safer alternatives in patients who have had a rash from aromatic anticonvulsants. However, in a 41-year-old man a fixed drug eruption that occurred after phenytoin and carbamazepine also occurred after valproate (SEDA-22, 83).

Valproate appears to increase the serum levels of diazepam lo-razepam and possibly midazolam, while clobazam appears to raise valproate levels. Clonazepam clearance may increase and valproate clearance decrease during concurrent use, and increased adverse effects have been seen. An isolated case describes sleepwalking in a patient taking valproate and zolpidem. 

In one study sodium valproate was reported to have no marked effeet on clobazam, but a study in children found that clobazam caused an 11% increase in the serum levels of valproate, despite a reduction of at least 10% in the valproate dosage. ... 

It has been recommended that if clobazam is added to valproate it would be prudent to monitor for any increases in valproate serum levels. ... 

The half-life of oxiracetam was shorter in patients taking car-bamazepine and valproate or clobazam, but oxiracetam did not affect the serum levels of sodium valproate, carbamazepine or... 

Stiripentol was given marketing authorization in the European Union on 4 January 2007 for use in conjunction with clobazam and valproate as adjunctive therapy of refractory generalized tonic-clonic seizures in patients with severe myoclonic epilepsy in infancy (Dravet s syndrome) whose seizures are not adequately controlled with clobazam and valproate [284 ]. 

Drag-drag interactions Care must be taken in using stiripentol as add-on therapy with carbamazepine, clobazam, pheno-barbital, phenytoin, and valproate, because stiripentol inhibits the metabolism of these drugs and/or their metabolites [286 ].
