Clear cell sarcoma (CCS) was formerly thought to represent a primary soft tissue counterpart of cutaneous malignant melanoma. Nonetheless, it is now known that CCS exhibits a consistent and characteristic t(12 22) chromosomal translocation that is not shared by mela-nocytic lesions of the skin. This neoplasm is typified by an epithelioid and/or spindle cell constituency, with a delicate fibrovascular stroma and clear or lightly eosinophilic cytoplasm. Melanin pigmentation may or may not be observed.

The pathologic distinction between primary sebaceous carcinoma and metastatic renal cell carcinoma (RCC an important differential diagnostic alternative with a clear-cell appearance) is similar to that made earlier in reference to sweat gland carcinomas. Slow evolution of a solitary skin tumor favors a primary lesion. In addition, immunostaining for podoplanin is effective in this context that determinant is consistently present in sebaceous tumors, whereas it is absent in RCC. On the other hand, EFN-a may also be involved in the activation of autoreactive T-cells as has been proposed for type I diabetes. An DFN-a inducible superantigen, encoded by the truncated envelope gene of a human endogenous retrovirus and specifically activating V 37 T-cells, has been detected in pancreatic lesions from type I diabetes patients, infiltrated by V 37 T-cells. Since IFN-a expression could be detected in pancreatic (3 cells in conceit with persistent viral infections, there is a clear link between viral infections and autoimmunity via IFN-a-stimulated superantigen expression.
